Use this form to request the temporary suspension of an employee's salary sacrifice vehicle lease payments during maternity, paternity or adoption leave where continued deductions would reduce their salary below the National Minimum Wage.
A minimum of six monthly lease payments must have been made before a suspension can be considered. If approved, payments will be suspended until the employee returns from leave (up to 12 months) or the lease ends, whichever occurs first.
By submitting this request, the employer is asking for the employee's monthly lease payments to be temporarily suspended in accordance with the scheme terms.
